How Do I Become A New Patient?

Having a baby? Thinking about transferring care? Want to learn more about our providers and tour the office? 


Just call our office to make your first appointment or set up a Meet and Greet!  Once you decide to join POBC, we will take care of the rest.

Our Goal Is To Provide The Best Medical Care

To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records (or at least vaccine records) before we can assume care and schedule an appointment. 


For Newborns

Be sure to bring the paperwork given to you at the hospital. 


For New Families Transferring To Us

When transferring a child to a new pediatric practice, it's essential to gather and organize all medical records, including vaccination records and any relevant test results. Communicating with both your old practice and us will help ensure a smooth transition of care, providing necessary information about the child's medical history and ongoing treatments.


Transferring Paperwork

If you are transferring from another practice, we’re here to help you obtain your prior medical records. Simply fill out our medical record request form, and we’ll handle the rest! Alternatively, you can request your records directly from your previous practice. Having a clear understanding of your child's prior care and diagnoses is essential for us to provide the best possible care moving forward.
